Willow Brook Christian Communities seeks the services of an independent nurse practitioner to perform face-to-face resident appointments/counseling & to gather/record clinical information to support diagnosis & treatment management while consulting with medical director/physicians/medical practitioners. Primary focus centered on hospital patient discharges into SNF or those requiring custodial care. NP will also perform staff employment physical exams. We’re seeking a professional with geriatric experience, at least four years as a nurse/nurse practitioner, and Master’s Degree in nursing. Interested individuals must hold a current NP license in Ohio.
Willow Brook’s three communities are centrally located in Delaware & Columbus & provide designated, private clinical space for appointments. Hours of service, fee schedule, and patient/insurance billing will be established at the nurse practitioner’s discretion.
